About the role This role is based withing the Hospital in the Home (HitH) team that operates out of the Medical Assessment Unit (MAU) so whilst the role is predominantly HitH there will be opportunity for shifts within the MAU. The HitH role will involve the identification and recruitment of patients in to the service linking closely with the wards as well as the MAU, ED and other frontline services. This will ensure that variety is a key element in this busy but rewarding role. The MAU works closely with ED to suport efficient patient flow out of the department and you will have the opportinity to work closely with our skilled teams including a designated Nurse Practitioner. The Hospital in the Home service Incorporates 5 'virtual medical beds' in the patients home to either avoid a hospital admission, or shorten their length of stay Provides Care for acute hospital level patients in the comfort of their own home Assess and treat patients who are still considered inpatient under hospital care. Liaise closely with the Medical Assessment Unit The MAU Streamlines the assessment and admission of non-critically ill medical patients Admits from ED in a timely manner to support the ED model of care and patient experience Manages the care of these patients for up to a 72 hour admission before discharged or transferred Shifts are 8-10 hours, rostered and rotating 24/7 – incl. night shits. About you Current registration with the New Zealand Nursing Council 3 Years experience in an acute setting Previous experience in NZ community nursing.
