Clinical Coordinator

Health Hawkes Bay – Hastings, North Island We are looking for a Clinical Coordinator to join our Te Rau Kawakawa Health Programmes team to support the coordination and delivery of delegated clinical programmes and services across primary care in Hawke’s Bay. Health Hawke’s Bay – Te Oranga o Te Matau-a-Māui is an established Primary Health Organisation (PHO) in Hawke’s Bay. Reporting to the Health Programmes Team Lead, the Clinical Coordinator will provide day-to-day coordination and clinical support across delegated health programmes and services. The role works closely with general practice teams, community pharmacy, primary care providers, contracted service providers and internal kaimahi to support programme delivery, pathway implementation, referral processes, quality improvement and provider engagement. In this role you will: Coordinate and support delegated clinical programmes, pathways and services in primary care. Support the implementation and socialisation of new health pathways, programmes and initiatives. Assist providers with programme access, referral processes, claiming support and operational guidance. Work with providers and internal stakeholders to support high-quality, safe and effective programme delivery. Maintain clear, auditable records and support compliance with programme criteria, contracts, policies and relevant legislation. Build and maintain positive relationships across the primary care network and wider health sector. Apply an equity lens to your work, with a strong commitment to Te Tiriti o Waitangi and improving outcomes for Māori and other priority populations. You will be a Registered Nurse with a current annual practicing certificate and at least two years’ nursing experience, ideally within primary health care. Experience supporting the delivery of health services or programmes would be an advantage. You will bring sound clinical judgement, strong relationship management skills, excellent written and verbal communication, and the ability to work collaboratively with a wide range of stakeholders. You will also have a strong understanding of health inequities in Aotearoa New Zealand, experience applying Te Tiriti o Waitangi in practice, and a commitment to continuous improvement. Strong digital literacy, analytical skills, and confidence working with data, reporting and systems will support your success in this role.
