District Nurse - Hawkes Bay Fallen Soldiers HospitalPermanent, Part -Time, 64 hours per fortnight (0.8 FTE)A rewarding opportunity to deliver care in the community, supporting patients and whānau to maintain independence and wellbeing in their own homes. About the Role As a Registered Nurse - District Nursing, you will provide high quality, patient centred care to people in their homes and community settings.You will undertake comprehensive assessments, develop and implement care plans, and deliver a wide range of nursing interventions to support patients with acute and long term conditions. A key focus of the role is health education, empowering patients and whānau to manage their health and maintain independence.You will work autonomously while also collaborating closely with the wider multidisciplinary team, including primary care, allied health and community providers, to ensure coordinated and effective care. The role also involves accurate documentation, use of workload management tools, and contribution to quality improvement and service development.This is an ideal role for a nurse who enjoys variety, independence and building strong relationships with patients and whānau in a community setting. Please click here to view the position description About the Team/Service/ Location You will be part of the Community Nursing Service based at Hawkes Bay Fallen Soldier Memorial Hospital, delivering care across the region.The team works closely with hospitals, primary care and community providers to support patients at home, with a strong focus on keeping people well in the community and reducing avoidable hospital admissions.For more information on living and working in Hawkes Bay clickHERE About you Current registration with the Nursing Council of New Zealand and a valid Annual Practising Certificate At least 3 years postgraduate clinical experience IV certification Ability to work both independently and within a multidisciplinary team Strong assessment, communication and organisational skills Confidence managing a varied clinical workload A current full NZ driver licence Commitment to Te Tiriti o Waitangi and equitable health outcomes Desired: Experience in community or district nursing Competence in wound care, compression bandaging and vascular assessment Knowledge of longterm condition management and primary care Experience with palliative care and catheter management Working towards or holding a PDRP portfolio and/or postgraduate qualification Working at Health New ZealandHealth New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of NewZealand.
