Location: New Plymouth, Taranaki Engagement: Full-time What you’ll do Conduct comprehensive assessments of patients with diabetes to understand their individual needs and health status. Develop personalized care plans in collaboration with patients, ensuring they are tailored to meet specific health goals and lifestyle preferences. Provide education and training to patients and their families on diabetes management, including insulin administration, dietary recommendations, and blood glucose monitoring. Collaborate with a multidisciplinary team, including endocrinologists, dietitians, and nurses, to ensure a holistic approach to diabetes care. Monitor patient progress and adjust care plans as necessary, utilizing evidence-based practices to enhance outcomes. Facilitate group education sessions and workshops to promote diabetes awareness and self-management skills within the community. Stay current with the latest research and advancements in diabetes care to provide patients with the most effective treatment options. Advocate for patients by connecting them with additional resources and support services, including mental health services and community programs. Maintain accurate and up-to-date patient records, ensuring compliance with healthcare regulations and standards. Participate in quality improvement initiatives aimed at enhancing diabetes care delivery within the organization. Engage in professional development opportunities to further enhance clinical skills and knowledge in diabetes management. Provide support and mentorship to junior staff and trainees, fostering a collaborative learning environment within the team. Requirements A relevant degree in nursing, dietetics, or a related health field, with a focus on diabetes care preferred. A minimum of three years of clinical experience in diabetes management or a related area. Current registration with the appropriate professional body in New Zealand. Strong understanding of diabetes pathophysiology, treatment options, and self-management strategies. Excellent communication skills, with the ability to convey complex information in an accessible manner to patients and their families. Proven ability to work effectively within a multidisciplinary team and foster collaborative relationships. Strong organizational skills and attention to detail, with the ability to manage multiple priorities effectively. Proficiency in using electronic health records and other healthcare technology. A commitment to ongoing professional development and staying informed about advancements in diabetes care. Nice to have Certification as a Diabetes Educator or equivalent qualification. Experience in community health or public health initiatives related to diabetes. Familiarity with culturally competent care practices, particularly in relation to Māori and Pasifika populations. Knowledge of motivational interviewing techniques and behavior change strategies. Experience in conducting research or quality improvement projects in diabetes care. Skills & tools The ideal candidate will possess strong clinical assessment skills, excellent interpersonal abilities, and a deep understanding of diabetes management. Familiarity with healthcare software and electronic medical records is essential, as is the ability to utilize data to inform patient care decisions. Proficiency in educational techniques and group facilitation will also be beneficial in this role.
