Case Manager

Te Mahi/About the Position Case Managers have an essential role in community care, liaising with clients and whānau to assess clients' health needs, and establish and review care plans. You will also work closely with others involved in the homecare team to ensure our services are delivered to a high standard, and with other health services involved in clients' care and support. We have a full-time permanent position available, usual hours of work will be Monday to Friday within business hours.
