Fixed-term opportunity commencing in 2026 and continuing until the end of 2027 Up to 36 hours per week, with hours and working days negotiable Are you someone who connects easily with people, takes initiative, and wants to make a practical difference in the health and wellbeing of your community? Aramoho Health Centre is seeking a compassionate, proactive Health Coach/Kaiāwhina to join our established and highly integrated primary care wellbeing team. Working alongside two Health Improvement Practitioners and the wider general practice team, you will support patients and whānau to identify what matters to them, build practical skills, navigate barriers, and make sustainable changes toward better health and wellbeing. This is a varied role that reaches beyond the consulting room. On any given day, you might receive a warm handover from a GP or nurse, meet with someone for a planned Health Coach appointment, reconnect with a patient who has disengaged from healthcare, facilitate a wellbeing group, visit someone at home, or help develop a new initiative in response to the needs of our practice population. What you’ll be doing You will work with patients and whānau of all ages and from diverse backgrounds. Your work may include: Building trusting relationships and completing holistic, person-centred assessments. Helping people identify their own goals and develop realistic plans for achieving them. Supporting self-management across areas such as emotional wellbeing, stress, sleep, physical activity, long-term health conditions, smoking, alcohol and other substance use, social connection, and healthy routines. Teaching practical coping, problem-solving, and health-management skills. Providing a mixture of warm handovers, booked appointments, active follow-up, outreach, and home-based support. Reconnecting with people who have experienced barriers to accessing healthcare or have disengaged from services. Helping patients and whānau navigate health, social, and community services. Supporting people to carry out plans developed with their healthcare providers. Contributing to walking groups, wellbeing groups, community activities, and other practice initiatives. Working with practice information to identify unmet health needs, access barriers, and opportunities for proactive population-health support. Developing new ideas collaboratively with the wellbeing and general practice teams. Maintaining timely, clear, and accurate electronic health records. Who we’re looking for Formal health qualifications and previous Health Coach experience are not required. We can provide the training - you will bring the attitude, relationships, and commitment. Understand the importance of Te Tiriti o Waitangi, cultural safety, and working respectfully with people’s different identities, values, and experiences Are proactive and can turn ideas into action while collaborating closely with others. Can encourage people to recognise their strengths, build confidence, and take achievable next steps. Are psychologically flexible and able to respond thoughtfully when plans or priorities change. Work well independently while knowing when to seek support, guidance, or clinical input. Communicate openly, contribute positively to a team, and respond constructively to feedback. Can maintain clear professional boundaries and sustainable self-care in a role involving people with diverse and sometimes complex needs. Are comfortable learning clinical computer systems, documenting your work, and using information to identify opportunities for proactive support. Experience in health, hauora Māori, Whānau Ora, community work, peer support, mental health, addictions, youth work, disability support, health promotion, social services, or another people-focused field would be valuable but is not essential. Relevant lived experience is also welcomed when it can be used thoughtfully and appropriately. Knowledge of Whanganui community organisations and support services, confidence with tikanga Māori, and some knowledge of te reo Māori would be advantageous. Supported from the beginning We want the successful applicant to begin as soon as practicable in 2026 so that they can complete a thorough, supported induction alongside our current Health Coach before taking over the role. Your induction and development will include: Shadowing and learning directly from the current Health Coach. Observing and working alongside our Health Improvement Practitioners. Opportunities to observe other Health Coaches working in the Whanganui community. Funded formal Health Coach training. Regular individual and team supervision. Access to relevant local and national Health Coach learning networks and professional development. About the position This is a fixed-term position commencing as soon as practicable in 2026 and continuing until the end of 2027. It provides cover while our current Health Coach completes clinical psychology internship training. The position offers up to 36 hours per week. The number of hours, working days, and how those hours are arranged are negotiable with the successful applicant. There is potential for the role to continue beyond the fixed term, subject to future service funding and contractual arrangements. The successful applicant will be based entirely at Aramoho Health Centre. They will work under the guidance of the Wellbeing Team Lead and report to the Practice Manager. Because the role may include outreach and home visits, applicants must hold a full, clean New Zealand driver licence and have access to a reliable vehicle. Appointment will alsobe subject to satisfactory police vetting and the required Children’s Worker Safety Checks.
