About the Role Permanent As a Registered Nurse, Hospital in the Home (HiTH), you will work within a virtual ward model that supports patients with acute or sub-acute conditions to receive safe, high-quality care outside of a traditional hospital setting. These positions offer a unique opportunity to work in an innovative and evolving model of care. You will provide comprehensive nursing assessments for patients deemed suitable for Hospital in the Home by a senior medical officer. Using telehealth, you will assess health history, clinical observations, diagnostic results, and identify patient needs, risks, and early warning signs that inform care planning and required interventions. You will take a proactive approach to patient management, ensuring timely escalation and coordination of care as required. About the Team/Service/ Location Our Hospital in the Home (HiTH) service operates as a virtual ward, providing seven-day coverage from 7.00 am to 10.00 pm. The service is fully supported by a Consultant and Resident Medical Officers, working collaboratively to ensure safe and effective patient care in the community. About you To be considered we will need the following: Current registration with the Nursing Council of New Zealand Strong clinical assessment skills and the ability to identify patient deterioration and risks Experience or confidence in delivering care via telehealth or virtual models Excellent communication skills and the ability to manage patient care proactively An understanding of holistic care, including psychosocial, cultural, and learning needs Working at Health New Zealand Health New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
