Aboutthe Role TheInfection Management Service | Te Whare Tukumate, based at ChristchurchHospital, provides specialist assessment, advice and management for people witha wide variety of infections caused by bacteria, viruses, fungi and parasites. Ourmultidisciplinary team includes medical and nursing specialists in infectiousdiseases and provides care to patients both within the hospital and across thewider community. Follow-up care may be provided in person, in OutpatientsDepartment or through virtual clinics. Wehave an opportunity for a Registered Nurse to join our specialist team in apermanent 0.6FTE position. Working collaboratively with nursing, medical andwider multidisciplinary colleagues, you will support the assessment, coordination,and delivery of high-quality care for patients accessing the InfectionManagement Service. Inthis role, you will: Undertake comprehensive nursing assessments and contribute to thedevelopment, implementation, and evaluation of individualised care plans tocontinue antibiotic treatment in the community, once discharged. Provide nursing care, education, and support to patients and theirwhānau Work collaboratively with the multidisciplinary team to facilitateand coordinate patient ongoing care, and discharge planning Work with district Nursing Service in the community, including inother regions, e.g., West Coast and North Canterbury to set up OPAT services. Recognise changes in patients' health status and respond orescalate appropriately Support patients to understand their treatment and maximiseopportunities for independence and self-management Maintain accurate clinical documentation and ensure patientprivacy and confidentiality Contribute to quality improvement, nursing education and theongoing development of clinical practice within the service Please note this role is open only to current staff ofHealth New Zealand. You Will bring sound nursing assessment and clinical decision-making skills, alongside a commitment to ongoing professionaldevelopment.
