About the Role We have an exciting opportunity for an experienced Senior Social Worker to join our Social Work team in a permanent, full-time (1.0 FTE) position. This role offers the opportunity to further develop your leadership capabilities while working across the Te Toka Tumai region with adults living with chronic and long-term health conditions. Combining advanced clinical practice with leadership development, this role provides opportunities to contribute to service development and quality improvement initiatives while supporting the delivery of high-quality, person-centred care in the community. Key responsibilities include: Working with adult clients in the community who have physical disabilities and complex health needs. Managing a clinical caseload and providing comprehensive, evidence-based social work assessment and intervention. Contributing collaboratively within a multidisciplinary team, including Allied Health professionals in the Community Allied Health Service, as well as colleagues from Community Rehabilitation (CoRe), the Intermediate Care Team (ICT), Needs Assessment Service Coordination (NASC), District Nursing, and Specialist Nursing Services. Providing supervision, mentorship, and support to junior Social Workers and/or other junior staff members in the wider team. Participating in service development and quality improvement initiatives within the community setting. Taking on delegated responsibilities from the Social Work Practice Supervisor or Service Lead as required. You will be supported by the Social Work Practice Supervisor (SW PS) and work alongside experienced interdisciplinary teams committed to delivering high-quality community services. The role may be based at either Greenlane Clinical Centre or Point Chevalier Clinical Centre, depending on service requirements. About the Team/Service/Location The Community Allied Health Team is a large multidisciplinary service for adult clients who have physical health and disability needs and live in the central Auckland region. Our Social Workers work in multidisciplinary locality teams and are committed to the holistic delivery of health services within the Te Toka Tumai|Auckland community providing integrated care that is coordinated around the client and their needs. About you Has a minimum of 7 years SOCIAL WORK PRACTICE experience. Is registered with the New Zealand Social Work Registration Board and holds a current Annual Practicing Certificate. Is an experienced practitioner who is able to demonstrate a senior or advance level of HEALTH SOCIAL WORK expertise. Has proven supervision, mentoring, coaching and leadership skills. Is motivated and passionate to ensure the delivery of high-quality HEALTH Social Work services. Preferably has experience in the community HEALTH setting. Has experience in working in a multidisciplinary team. Holds a current clean drivers licence. For more information on living and working in Auckland.
