Gateway Assessment Coordinator: Taranaki Base Hospital: Permanent Part-time (16 hours per week) Make a difference to outcomes for children and young people through coordination of a health and wellbeing assessment. About the Role The Gateway Assessment Coordinator receives and manages referrals for a comprehensive health and developmental assessment for children and young people connected to Oranga Tamariki. The coordinator is responsible for gathering birth and health information from primary and secondary care providers as well as a learning profile from early childhood centres, kindergartens and schools. Scheduling of appointments to facilitate timely assessment is an important part of the role and provision of a summary/report with any recommendations for health or learning is made available to Oranga Tamariki staff working with the child, young person and family. About the Team/Service/ Location The Gateway Assessment Coordinator is part of the community child health team based at the Child and Adolescent Community Centre at Taranaki Base Hospital. The coordinator works closely with the hospital liaison social worker, the clinic coordinator, and paediatricians that participate in the gateway assessment process.
