As a CREST Support Worker, you’ll play a vital role in delivering rehabilitation support to tangata in their own homes. Working as part of a multidisciplinary team, you’ll help people transition safely from hospital back into the community, supporting recovery, independence, and wellbeing. This is a fixed-term role. He kōrerowhakamāramamōtēneitūranga| About the role Each Tangata will have high risk individual care requirements, and you will be responsible for delivering rehabilitation programmes under the direction of Registered Nurses. You will provide safe, respectful, and person‑centred support in people’s homes, being mindful of what matters to them, including their culture, values, beliefs, routines, and boundaries. This is a 7 day a week service, so flexible availability is necessary. Cares will include: Spinalcares Rehabilitative care Mental health Medication Whakangungu | Training We provide practical and online training during the first few weeks of employment. With the added bonus of year-round training with regular refreshers and new modules to keep skills up to date. You’ll be supported through structured induction, ongoing training, refreshers, and clear development pathways. Ngātohungatangamōtēneitūranga| Skills and experience Hold a Level 3 or Level 4 Health and Wellbeing qualification or have equivalent, relevant experience Spinal injury Stoma Management Te Tiritio Waitangi and Bi-cultural approach Experience issuing prescribed medication Full/restricted driver's license The ability to drive up to an hour to and from your tangata The right to work in New Zealand Be willing to start work at 7am Be available to workweekends,eveningsand split shifts Provide flexibility to sleepover orwakeover Be willing to learn, reflect, and adapt your approach to what matters for each person Have experience in or exposure to caregiving, disability support, aged care, community work, or other people‑facing roles Committed to culturally safe, respectful support for people from all backgrounds Intuitive and responsive to people’s needs He aha ai mātou? | Why Us Permanent hours – mornings, evenings, weekends Support through structured induction, ongoing training, refreshers, and clear development pathways.
