The Health of Older People's Service (HOPS) is seeking acompassionate, skilled Clinical Assessor to join our Complex Clinical CareNetwork (CCCN) team. This role is central to enhancing the wellbeing, self-determination,and quality of life of our older population through comprehensive assessment,restorative care planning, and coordinated support. About the Role As a Clinical Assessor, you will complete holistic clinicalassessments using InterRAI Home Care or Contact Assessment tools, identifyingmedical, rehabilitation and support needs for older people across the WestCoast. You will work collaboratively with clients, whānau, and the CCCNinterdisciplinary team to prioritise needs and develop restorative,evidence-based Care Plans. Key responsibilities include: Clinical Assessment & Care Planning Identifyclient needs that promote independence and self-determination. Developrestorative care plans using Post Assessment Guidelines. Consultwith clients, whānau, caregivers, GPs and service providers as required. Documentassessments, outcomes, and care plans accurately and promptly. Care Coordination Facilitatereferrals to Allied Health and relevant community services. CoordinateSupport Plans, ensuring service options meet client needs. Reviewand adjust service packages to maintain appropriateness and effectiveness. Provideclients with information about available services, choice of provider, andcosts where applicable. Professional Practice Workwithin your professional scope, providing discipline-specificinterventions (e.g., OT tasks, nursing input, social work support) asappropriate. Maintainhigh standards of cultural safety and bi-cultural practice. Participatein the Single Point of Referral triage system. Completestatistical and administrative reporting requirements. Workcollaboratively as part of the interdisciplinary CCCN team. Maintainstrong communication pathways with GPs, service providers, and communityagencies. Representthe service professionally at meetings and client conferences. Professional Development Maintaincurrency of clinical knowledge in gerontology and the ageing process. Participatein ongoing training, supervision, and annual performance reviews. Supportthe learning of students and colleagues as appropriate. Health, Safety & Emergency Procedures Complywith Te Tai o Poutini | Health New Zealand | Te Whatu Ora health andsafety policies. Identifyand report hazards, incidents, and clinical risks. Maintaincompetencies in emergency procedures, CPR, and safe manual handling. About You Essential Qualifications & Experience Arecognised professional qualification (e.g., Nursing, Social Work,Occupational Therapy) and/or registration under the HPCA Act or relevantprofessional body. CurrentAnnual Practising Certificate (or equivalent for the discipline). Commitmentto completing and maintaining InterRAI competency (or willingness tocomplete training). FullNZ driver's licence and ability to drive manual and automatic vehicles asroad travel will be expected. Strongclinical judgement, assessment skills, and understanding of the ageingprocess. Experiencein gerontology or specialist older-adult care. Knowledgeof mental health or health changes affecting older people. Understandingof community services, access pathways, and eligibility criteria.
