Health Promoter - Kaiāwhina

The Health Promoter – Kaiāwhina will be a key part of the engagement and coordination of our immunisation service — connecting with whānau, understanding barriers, arranging practical support and working alongside our Vaccinator and wider clinical team to help turn engagement into completed immunisations. At the heart of this role is making sure whānau aren't simply sitting on an overdue list. They're being contacted, barriers are understood and addressed, appointments are being made and supported, and more tamariki are accessing the immunisations they need. Key responsibilities Contacting and following up whānau who are overdue for immunisations Building trusted relationships and having respectful, informed conversations about immunisation Identifying barriers that may be preventing whānau from accessing vaccination Coordinating appointments, outreach visits, transport and other practical support Working alongside the Vaccinator at clinics, home visits, community events and outreach activities Delivering health promotion, whānau education and community engagement activities Maintaining accurate records and supporting service monitoring and reporting Undertaking immunisation training and development, with the potential to progress toward Vaccinating Health Worker capability Supporting maternity, well child and other clinical or population health activities when immunisation service requirements allow About you Has an exceptional ability to connect and build trusted relationships with whānau Is confident having conversations about immunisation without judgement or pressure Is organised, proactive and persistent with follow-up Is comfortable working alongside whānau who may experience barriers to accessing healthcare Has strong communication and health-promotion skills Understands and is committed to kaupapa Māori approaches and working effectively with whānau and the Ōtaki community Can work effectively alongside clinical kaimahi while understanding the boundaries of their role Is willing to undertake further immunisation training and development Experience in whānau engagement, outreach, health promotion, hauora or community-based services would be an advantage About us Te Puna Oranga o Ōtaki is a kaupapa Māori health and social service provider based in Ōtaki.
