East Health Services – Howick, North Island Located near the heart of Howick, Crawford Medical is seeking a Health Coach to join the team, in an exciting service at the forefront of change to improve the health and wellbeing of our communities. This integrated model of care expands the general practice team to include Health Improvement Practitioners (HIPs) and Health Coaches, increasing access for patients to self-management support and social and cultural supports; working alongside patients to help them build knowledge, skills and confidence to lead their own health and wellbeing. Health Coaches work in partnership with the patient and their family/whānau to: Help them understand the general practice team advice Find ways to use that advice to improve their health As a core member of the general practice team, the Health Coach works with individuals and family/whānau to provide ongoing support via telephone, video, and face to face contact. A large component of this role will be to facilitate groups and assist those requiring support in navigating health and wellbeing providers. Health Coaches also work collaboratively with other members of the team including HIPs, to provide outreach to enhance engagement with patients who are “hard to reach”. Whilst there are some specific technical skills that Health Coaches are trained to use, the successful applicant will have: A motivating attitude and approach Experience working in primary care with patients as part of the health care team The ability to connect with a wide range of people from various backgrounds and cultures An empathic, caring and nurturing approach Legal rights to work in New Zealand This is a permanent role for 40 hours per week from Monday to Friday and we offer full specialist training and support. #J-18808-Ljbffr
