Whānau Ora Targeted Support & Community Capability Lead Make a practical difference for Pacific whānau Tongan Health Society (THS) is seeking a skilled and culturally grounded practitioner to lead two connected Whānau Ora pathways: Targeted Support Services and Community Based Capability Building. This is a hands-on role for someone who can turn whānau goals into practical action, remove barriers, connect services and build capability in trusted community settings. Targeted Support is not general case management. It is activated when an existing Whānau Plan has stalled because a specific barrier cannot be resolved through usual Navigator support. The role will coordinate a defined, time-bound intervention and then hand the matter back to the Navigator once the barrier is resolved, reduced or appropriately escalated. Community Based Capability Building works earlier. It supports prevention, youth leadership, family capability, cultural wellbeing and stronger community connections through trusted settings such as churches, community groups and local networks. What you will do Receive and triage Targeted Support referrals linked to active Whānau Plans. Work with whānau and Navigators to define the barrier, intervention, outcome and timeframe. Coordinate practical responses across housing, employment, income, education, family wellbeing, health and mental health pathways. Maintain clear boundaries between Targeted Support, Navigator Services, Flexi Fund and other THS contracts. Coordinate community capability activities including youth aspiration, family capability, cultural wellbeing and community champion development. Build strong relationships with Tātou network partners, government agencies, community organisations, churches and specialist services. Monitor outcomes, contribute to reporting and case studies, and use learning to improve the service. Identify and elevate safeguarding, privacy, conflict-of-interest, clinical and reputational risks. What success looks like Whānau receive timely support when a specific barrier is stopping progress. Interventions are targeted, measurable and time-bound, with a clear hand-back to the Navigator. Whānau plans move from planning into action and barriers are demonstrably reduced or resolved. Pacific families access earlier support through trusted community settings before issues elevate. Youth, families and community leaders build confidence, capability, cultural connection and stronger pathways to support. Referrals, expenditure and outcomes are clearly documented and auditable. About you Experience in Pacific health, social services, Whānau Ora, community development, youth work, housing, employment, mental health or a related field. Strong understanding of Pacific communities and culturally responsive, whānau-centred practice. Excellent practical problem-solving, coordination and relationship-management skills. Confidence working across agencies and navigating complex service systems. Sound judgement in complex situations, including safeguarding, privacy and conflicts of interest. Strong documentation, digital literacy and outcome-reporting skills. A relevant tertiary qualification is preferred. Tongan language capability and experience working with Tongan and wider Pacific whānau are highly desirable. Why join THS?
