Pharmacy Care Group Limited – Auckland, North Island Provide comprehensive medication management services for aged care residents, including review of medication profiles, identification of drug interactions, contraindications, duplication, and inappropriate medicines. Assess medication appropriateness, effectiveness, safety and adherence, and support deprescribing where clinically indicated in consultation with the resident's GP and care team. Key responsibilities Review residents' medication profiles and identify opportunities to improve medication therapy Identify potential drug interactions, contraindications, duplication, inappropriate medicines and medication-related risks Assess medication appropriateness, effectiveness, safety and adherence Support appropriate deprescribing where clinically indicated and in consultation with the resident's GP and care team Monitor residents who require additional medication-related clinical support Provide recommendations to prescribers regarding medication therapy Support the safe use of high-risk medicines in aged care Assist with medication management for residents transitioning between hospital, aged care and community settings Promote a strong medication safety culture within supported aged care facilities Monitor medication incidents and assist with identifying trends and opportunities for improvement About you Work within the requirements of the Pharmacy Council of New Zealand, relevant New Zealand legislation, professional standards and organisational policies Maintain appropriate clinical documentation and records Maintain professional competence and undertake continuing professional development relevant to aged care pharmacy #J-18808-Ljbffr
