We are seeking an experienced and compassionate New Zealand Registered Nurse to provide high-quality clinical care and support to patients in their own homes. This is an autonomous and rewarding community-based role where you will use your practical nursing knowledge, assessment skills and clinical judgement to support patients throughout their care journey. You will have the opportunity to build strong relationships with patients, their whanau, GPs and other healthcare referrers while providing continuity of care from beginning to end. Additional birthday leave Opportunity to expand your skill set and advance your career Key Responsibilities: Complete comprehensive patient assessments in the community Deliver safe, evidence-based nursing care in patients' homes Provide wound assessment, treatment and ongoing wound management Coordinate patient care and manage an assigned caseload Educate patients and their whānau about treatment plans and self-management Monitor patient progress and respond appropriately to changes in condition Maintain accurate and timely clinical documentation Develop positive working relationships with GP practices, specialists and other referral partners Skills and Experience: Current NZ Registered Nurse registration and Annual Practising Certificate (APC) Wound management experience, and experience in burns management and urinary catheter care Experience in community care, or a strong understanding of community care (preferred) Caring, empathetic nature and the ability to connect with people from diverse backgrounds Full NZ driver's license and have access to a reliable vehicle Reference: 220128 #J-18808-Ljbffr
