Health New Zealand - Te Whatu Ora - Southern seeks a Case Manager (CART) for Primary and Community to support older adults in Wanganui. The role focuses on in-home assessments to determine independence and coordinate care with clients, whanau, and local services. You will join a multi-disciplinary rehabilitation team to provide clinical assessments and link clients to services that maintain independence in the community.
