Registered Nurse -Outreach

Provide community-based care beyond a traditional clinic environment Full time role, Monday to Friday, core hours Access external supervision, wellbeing leave & ongoing professional development Role Summary Deliver outreach healthcare to people experiencing homelessness, financial hardship and significant barriers to care across central Auckland. Based in Central Auckland, with outreach work undertaken in community settings. Working independently and within a multidisciplinary service, you will develop trusted relationships with clients and whānau while supporting their health goals. You will collaborate with GPs, a Nurse Practitioner, Social Workers, a Health Coach, housing professionals and other community providers. Key Responsibilities Deliver holistic nursing care in community settings using culturally responsive and trauma-informed practice Establish respectful relationships that encourage ongoing engagement with healthcare Coordinate care with general practice, primary health and community services Arrange referrals to health, social and specialist providers Help clients understand their options, make informed decisions and manage their health where possible Apply current clinical knowledge, reflective practice and sound professional judgement About You Registered with the Nursing Council of New Zealand and holding a current Annual Practising Certificate At least three to four years of recent nursing experience Ideally experienced in primary care, community health or outreach nursing Confident supporting people affected by mental health challenges, alcohol or other drug use, homelessness, hardship or social deprivation Experienced working with Māori and/or Pacific communities Knowledgeable about trauma-informed care and able to apply it in practice Committed to Te Tiriti o Waitangi and equitable health outcomes Comfortable working both autonomously and collaboratively Hold a full, clean New Zealand driver licence Relevant experience gained through paid employment, voluntary work, marae, iwi, church, whānau or other community responsibilities will be valued.
