Deliver hospital care while helping patients in their own homes. The Intermediate Care Team is part of the Community and Long-term Conditions Directorate and comprises the Hospital in the Home (HiTH), Rapid Community Access Team (RCAT), and the Interim Care Scheme (ICP). Being part of the team, you will deliver care through a ground-breaking model of care delivering hospital-level treatment at home, work with a highly skilled multidisciplinary team and supportive SMO colleagues, play a key role in shaping the future of virtual healthcare in Aotearoa, enjoy professional growth, leadership opportunities, and experience a dynamic role where no two days are the same. Responsibilities Deliver hospital-level treatment at home, support patients' recovery, and collaborate with multidisciplinary teams.
