Active+ is seeking an Integrated Care Pathway Clinical Manager in Auckland to lead and shape the ICPMSK service. You’ll review referrals, perform clinical triage, and mentor the Navigation Team while acting as a clinical expert for complex cases and pathway decisions. The role blends hands-on pathway navigation with client advocacy, coordinating with ACC, providers and GPs to ensure high-quality, culturally safe care.
