If that resonates with you, we'd love to talk. About the Role Based in Taradale, you'll support 22 people who live across five homes. Every person is different. Some have complex physical disabilities, others have communication, sensory or cognitive impairments. Every individual has their own goals, preferences and way of communicating. You'll work alongside experienced support workers, families, GPs, hospital teams and specialists to ensure each person receives the healthcare and support they need while maintaining as much independence, choice and control as possible. Your responsibilities include: Clinical assessment and ongoing nursing oversight Developing individualised health and risk plans Coordinating multidisciplinary care Coaching and supporting medication competent support workers Advocating with health services to ensure people receive equitable care Building trusted relationships with whānau and external providers Participating in family and MDT meetings Monday-Friday role with shared on-call (approximately one week in four) Who We're Looking For We're open to nurses from a range of backgrounds.
