Silverdale Medical Partnership – Silverdale, North Island Silverdale Medical Limited (SML) is offering a full-time Practice Pharmacist Prescriber role, available as either an employed position or an independent contractor arrangement. We are building a new interdisciplinary team model of care, and this role sits at its centre. You will be the medicines expert within the team, working directly with patients to provide individualised pharmacotherapy guidance, deliver education to patients and whānau, and support safe, effective prescribing across our practices and helping define what this new way of working looks like as we grow it. About the Role Prescribing, medicines management, optimisation, and reconciliation for patients Clinical evaluation and implementation of best-practice prescribing Medication audit and quality improvement Developing standards, policies, and procedures for safe and effective medicine use across our organisation Full participation in an interdisciplinary team structure, helping shape how that team works Building relationships with other health and wellbeing providers in the community We are Looking For Pharmacists Who: Hold a current APC with no restrictions Have 4+ years' experience as a registered prescribing pharmacist Are able to adapt and adopt new ways of working Have business/commercial acumen Are confident prescribing new items and developing new or multiple scopes/areas of expertise Understand health inequities in Aotearoa New Zealand and the application of Te Tiriti o Waitangi in primary care Bring cultural competency and confidence working with diverse communities Have experience in collaborative, multi-disciplinary health teams Have experience in primary care Are experienced supporting long‑term condition management through initiating and continuing prescribed treatment #J-18808-Ljbffr
