The Kaihapai Oranga Ngatahi (Physical Health Outcomes) supports tangata whaiora to improve their physical health and wellbeing through personalised exercise, nutrition, and healthy lifestyle programmes. Working alongside tangata whaiora and Mental Health Keyworkers, you will promote positive health outcomes by addressing the impacts of psychotropic medication, and socioeconomic challenges. This role involves assessment, planning, monitoring, education, and ongoing support to help tangata whaiora achieve and sustain their health goals. Kia ekena a angitū | To promote success in this role, you will have A National Certificate in Fitness (Level 4) essential. A full, clean New Zealand Driver Licence. You will have experience in personal training, individual and group fitness instruction, and working with people experiencing mental distress and associated disorders. You will demonstrate a proven ability to improve health outcomes for tangata whaiora and work effectively both independently and as part of a multidisciplinary team. Key responsibilities include: Conduct comprehensive health assessments, including monitoring weight, blood glucose, blood lipids, and smoking status. Develop tailored short-term health and wellbeing plans in partnership with tangata whaiora that align with their goals and aspirations. Deliver fitness, health education, and wellbeing interventions, including Te Ao Māori approaches where appropriate. Support tangata whaiora to maintain health gains beyond programme completion through connections with community-based supports and services. Coordinate activities, programmes, and venues to ensure consistent and effective service delivery. Record, report, document, and interpret outcomes in collaboration with tangata whaiora, keyworkers, and wider kaimahi to ensure continuity of care and safe practice. Collect and review tangata whaiora feedback to support programme evaluation and continuous improvement. Manage resources effectively while contributing to quality improvement, health and safety initiatives, and ongoing professional development. Demonstrate an understanding of Māori health perspectives and a commitment to culturally responsive practice. The successful candidate will be highly organised, relationship-focused, and passionate about supporting tangata whaiora to achieve sustainable health and wellbeing outcomes.
