The Role In this rewarding role, you will step out of the hospital clinic and into the homes in our community. You will focus on: Whānau Health Assessments: Conducting clinical, home-based assessments for age-related disabilities to connect whānau with essential support services. Preventative Care: Leading community health checks and screening for long-term health conditions. Improving Access: Actively engaging with whānau Māori to break down barriers to healthcare and ensure equitable health outcomes. Clinical Guidance: Providing everyday clinical safety support and guidance to our care teams. What you will bring: A current New Zealand Annual Practising Certificate (APC). A minimum of 2 years of nursing experience (ideally in community, primary care, or aged care). A current, clean New Zealand driver’s licence. A genuine understanding of, or a strong desire to learn and practice, tikanga Māori. Strong clinical assessment skills and a warm, relationship-focused approach. Knowledge of home-based support services and NASC assessment processes an advantage Completed or commitment to studying towards a post-graduate certificate in Nursing Key Relationships Whānau and community members Kaumātua and kuia Internal clinical and cultural teams GPs, allied health providers, and aged care services Iwi/Māori health partners and community groups Home support coordinators and community support workers Uphold and practice the values of the Trust including tu tika tu pono tu aroha, rangatiratanga, kaitiakitanga, whanaungatanga, manaakitanga and matauranga.
