As a Clinical Nurse Specialist, you will provide advanced clinical expertise, leadership and education to support the delivery of high quality, person-centred care for older adults at Whangarei Hospital.

You will work collaboratively within a multidisciplinary team to improve patient outcomes, promote best practice and support service development.

• Undertake assessment and interventions with elder clients to prevent admissions, support early discharge and maintain clients in their own environment.
\n
• Be part of the Acute Community Elder specialist nursing team, working with Acute Care of the Elder ED GNS, nurse practitioner, geriatricians and clinic Enrolled Nurse
\n
• Participate in strengthening and developing community specialist support models through Aged Residential Care.
\n
• Work with wider primary and community care services to facilitate integrated services for Elder people, to supporting optimisation of elder clients in their own environment home and assistance with avoiding unnecessary hospital admissions.

• Participate in developing pathways to provide timely access to specialist nursing and medical advice (Dementia pathway, ACP, falls).

This role is based at Whangarei Hospital and is part time working 32 hours rostered and includes some weekend working.

• Registration with the Nursing Council of New Zealand and a current Annual Practicing Certificate (APC)
\n
• A current Expert Registered Nurse Professional Development and Recognition Programme (PDRP) Portfolio
\n
• Postgraduate diploma in specialty nursing practice, working towards or willing to undertake.
\n
• Specialty knowledge and practise expertise
\n
• A personal commitment to ongoing learning and development
\n
• Communicates information effectively and accurately, both orally and in writing
\n
• Collaborates with fellow team members and work groups to achieve specific objectives.
